To answer your question, yes I made the entire cosplay (except the pants, boots, and dress shirt) myself. The cosplay was designed from scratch by analyzing the proportions from the TV series (what? it was 2 engineers in college working on it. You should have seen the board of calculations as my friend tried to figure out a french curve for the sleeve top

). As such I really can't give you any pattern names or numbers to base yours off of. However, I might be able to dig out the patterns and take pictures to give you can idea of how to make it.
Some of the concepts to defy physics like many cosplays tend to do took us a while to figure out (like the tall, open collar that somehow remained almost closed in the front with NO clasp). Oh, and if you compared the cosplay I made to some others, you'll notice that I made it out of a heavier material and darker in color. The reason behind that is because I proposed that we make it like a real military outfit. Class A formal uniforms are thicker and darker than the standard Class B's (I was in JROTC for 4 years) and it turned out pretty well using that fact.
